I acknowledge that I am purchasing either an in person mobile service or a self-administered teeth whitening kit that is designed to whiten the colour of my teeth. As a part of the purchase, I am asking for assistance in the use of my teeth whitening kit, and I intend that I will be allowed to use a specially designed LED lamp in order to accelerate the whitening process.
Although most natural teeth can benefit from teeth whitening treatment, I understand that everyone’s teeth are different and that results will vary.
Treatments/services are not transferable to other individuals.
CAUTIONS AND DISCLAIMER agreeing to this consent form frees all distributors, manufacturers and affiliates from all liability associated with the use of this formulation or system or additional use of any light source.
- Do not use if pregnant or lactating.
- Do not use if periodontal disease or other medical conditions exist in the mouth.
- Do not use if you have a history of allergic reactions to peroxide products.
- Do not use chew or use tobacco or drink alcohol during our treatment.
- Not suitable for children under the age of 16 years or any persons having known peroxides allergies.
- Whitening treatments are not effective on veneers, crowns or fillings.
Temporary side effects of tooth sensitivity or mild tissue irritation may occur but are usually transient and may last for a day or two. By agreeing to these terms and conditions, I indicate that I am eligible as per the criteria listed above, that I have read and fully understand this entire document and that I am performing this treatment under my own responsibility.

Returns & Refunds
Please select carefully as we do not accept refunds or exchanges on any of the cosmetic products listed on https://www.whitersmilesdental.com.au or on any third-party site. Whiter Smiles Dental will only offer clients a refund if we deem items to be faulty but do not if the damage is caused by the customer. An email to info@whitersmilesdental.com.au must be made within 48 hours of the product/s being delivered. If deemed faulty by Whiter Smiles Dental the customer must return the product/s within seven days of delivery.
Shipping
Whiter Smiles Dental does not hold any responsibility for goods that are lost or damaged in transit.

For professional teeth whitening treatment
This information is provided to give you insight into professional teeth whitening. Your cooperation and understanding of this material is necessary as we strive to achieve the best results for you.
Procedure
Professional teeth whitening is a treatment procedure available to lighten the colour of your teeth using a hydrogen peroxide mixture. It is a method use to achieve maximum whitening results in the shortest possible time with minimum sensitivity. During the procedure, the whitening gel will be applied to your teeth for two to four eight-minute sessions. For the duration of the entire treatment, a plastic cheek retractor will be placed in your mouth to help keep it open and for your teeth to be visible. Your gums will be covered with a barrier to ensure isolation from the hydrogen peroxide gel. Before and after the treatment, the shade of your teeth will be assessed and recorded.
Risks
All forms of dental or health treatments including teeth whitening have some risks and limitations. Complications that can occur in professional teeth whitening are infrequent and are usually minor in nature.
- Tooth sensitivity: During the whitening process some patients may experience tooth sensitivity. This is normal and generally mild if your teeth are not normally sensitive. If your teeth are normally sensitive, please inform us before treatment. Please let us know if you experience any discomfort during or after the procedure so we are able to help minimise your discomfort. A mild analgesic will usually be effective in eliminating any discomfort after your treatment if required.
- Gum and soft tissue irritation: Teeth whitening may cause inflammation of your gums, lips or cheek margins. This is generally the result of the whitening gel coming into contact with these tissues. Protective materials are placed in the mouth to prevent this, but despite our best efforts, it can still rarely occur. If any irritation does occur, it is generally short in duration and is very mild. Rinsing with warm salty water can relieve and repair any discomfort.
Expectations
Significant teeth whitening can be achieved in many cases, but there is no absolute way to predict how light your teeth will get. Please understand that teeth with multiple colourations, bands, splotches, blemishes or spots due to tetracycline staining or fluorosis do not whiten as well and may appear more spotted after treatment. These effects are generally short in duration. Professional teeth whitening is not recommended for pregnant or lactating women, children under 16 years of age or any persons having known peroxides allergies.
I understand that my perceived professional whitening treatment results cannot be guaranteed. I understand that after treatment, I will be required to refrain from consuming any chromogenic substances (i.e., tomato sauce, coffee, all tobacco products) for 48 hours.
